All new students must fill out the following forms before you may attend class.   Please download, print, and fill out the forms before you come to class for the first time.   Both forms must be notarized before students may participate in karate.    Your local bank can notarize the forms, at no charge.  Also, the BEP church administrator can notarize your forms at the BEP Church office during business hours.   Please call 410-626-8122 to reach the church office.
